PDF versions, print them, fill them out, and send them to us.
- If you're a
parent or guardian and you wish to enroll your child
as a MusicLink student, please complete the
Parent/Student Information Form
and the Policy Form.
- If you're a
classroom teacher or other community leader
and wish to nominate a student who you believe deserves to
participate in the MusicLink program, please complete the
MusicLink Nomination Form and Potential Talent Profile.
- If you're a
music teacher and you currently have students whom
you teach for free or for reduced fees, please complete the
MusicLink Registration Form and the
Potential Talent Profile.
- If you're a
music teacher and you'd like to volunteer to teach
a student who lives near you for free or for reduced fees, please
complete the Teacher Volunteer Form.
- If you're a
community leader and you'd like to assist MusicLink
by coordinating a local program or assisting a current coordinator
in your area, please complete the Community Volunteer
Form.
- If you're a
business that is interested in supporting MusicLink
teachers and students locally, regionally, or nationally, please
contact us and/or complete our
Business Partner Registration Form.
- If you have
an instrument that you would like to donate to the MusicLink
Foundation for use by our MusicLink students and programs, please
contact us through email, explaining specifics
of the instrument and your location.
- If you would
like to sponsor or coordinate a fundraiser to benefit
MusicLink students, please contact us.

Parent/Student Information and Policy Forms
Our Parent/Student Information Form provides parents the
opportunity to assess their childen's musical interests and
abilities. Our Policy Form explains MusicLink policies and
provides parents with the opportunity to authorize participation
in the program.

Musiclink Nomination Form and Potential Talent Profile
Our MusicLink Nomination Form is used to gather
basic information about the student who is being nominated (contact
information, age, gender, instrument, etc.).
Nominating a student to MusicLink is easy! Simply fill out the electronic
MusicLink Nomination Form provided below and submit it online,
or print out the PDF
version and mail to MusicLink Foundation, 1043 N. McKinley Rd., Arlington,
VA 22205. MusicLink will contact the nominator to confirm receipt.
MusicLink students:
- range from preschool age to seniors in high school
- show potential in music and interest in music lessons
- have financial need as shown by one of the following:
- family is eligible for the free or reduced lunch program
- family income is less than $42,000 for a family of four
- other financial circumstances show indication of need
Teachers who nominate students must notify the students and
parents/guardians of the nomination. Be sure to tell them that a
MusicLink representative will be contacting them soon.
Our Potential Talent Profile provides information to
MusicLink teachers about the musical interests, commitment,
and capabilities of the student.

MusicLink Registration Form
Our MusicLink Registration Form is for teachers who currently have
students on partial (half or less of the normal fee) to full
scholarship. Complete this form to join our team of MusicLink teachers
and you can take advantage of:
- Discounts and free music from major publishers
- Camp scholarships for your MusicLink student(s)
- Grass Roots Grants for alliance partner organizations
- Teacher Reimbursement Grants to assist in materials and music
- Assistance in providing instruments for your MusicLink student
- Communication with teachers nationally who share your interest
in reaching students in need through music.

Teacher Volunteer Form
Our Teacher Volunteer form is for music teachers who are interested
in joining the MusicLink team but do not currently have any students on
full or partial scholarship. Complete this form and you will be contacted
by a MusicLink representative.

Community Volunteer Form
Our Community Volunteer Form is for teachers and community
leaders who are interested in coordinating a local MusicLink program,
assisting our current coordinators, or helping raise funds to bring
a MusicLink program to your community.

|
Business Partner Registration Form
The Business Partner Registration Form is for businesses who
are interested in supporting MusicLink teachers and students locally,
regionally, or nationally. You may either contact us or complete the
form.
